Client Information & Counseling Policies
Please read this document carefully, since its provisions set the framework for your counseling. At you initial visit, your therapist can address any questions you may have about your policies.

Client Consent & Service Contract Adults
You will be asked to sign this form after reviewing our policies with your therapist. All adults who receive our services will have to sign.

Client Questionnaire
The questionnaire serves two purposes: to help you think about the concerns you may want to address in counseling; and to give your therapist a broad understanding of your overall situation.
Please take the time to fill out this form. We ask that you return the completed client questionnaire to our office before your first visit, if possible, or bring it with you for your first appointment. All the information you provide is confidential. The completed questionnaire will give your therapist necessary background information without using valuable time during your counseling visit.
If you are seeking couples or family therapy we ask that each participating adult fill out a separate questionnaire.

Minors Policies And Service Contract
Please read these policies carefully. Any questions you might have can be answered during your initial session.
